The post holder will undertake autonomous work across the acute medical, frailty and older person's wards and have the opportunity to work in the emergency department. The post holder's team would also cover areas, such as a frailty same day emergency care unit, and support the work of an assistant practitioner. There would also be opportunity to learn skills about critical care and may be required to cover this specialist area, depending on service demands. To take an active role in supervision of the team under the leadership of the Dietetic Team Lead, deputising in their absence. Other responsibilities may include attending and actively contributing at MDT meetings, board rounds to discuss patient cases, helping prevent inpatient admissions through intervention in the emergency department and discussing patient care, dietetic intervention, goals and discharge planning. The dietetic team has 14 dietitians, 4 dietetic assistants and 1 dietetic apprentice. The dietetic team is divided into sub-team depending on speciality and paediatrics is also part of the acute service. Junior dietitians have the opportunity to rotate between teams and the post-holder will be part of their rotational training. To work as part of an Acute Dietetic Team undertaking assessment of patients including those with diverse and complex presentations and conditions: using clinical reasoning skills and assessment techniques to formulate individualised treatment plans and programmes. To deliver individualised dietetic treatment programmes using a broad range of skills in individual and group setting. To continually reassess patient progress, adapting treatment plans accordingly, and developing discharge plans. To refer patient appropriately to other health care services according to protocol, providing timely and detailed information to allow continuity of care. To communicate effectively with patients, carers and other healthcare professionals using a wide range of verbal and non-verbal communication tools, share findings, agree treatment plans, progress nutrition, provide advice and teaching. This will include patients with communication and comprehension difficulties, and those who may be unable to accept their diagnosis / prognosis.
