Greater Manchester Mental Health NHS Foundation Trust is seeking a skilled and dynamic Perinatal Outreach Practitioner to join their new Specialist Perinatal Outreach Team in Manchester. This role focuses on providing outreach services for individuals in the perinatal period across the Greater Manchester area, bridging the gap between inpatient care and community services. Responsibilities include managing a personal caseload, conducting assessments, formulating care plans, and providing support for complex cases to ensure the well-being of both mothers/birthing persons and their babies.

The ideal candidate should be a qualified registered nurse, occupational therapist, or social worker with a passion for perinatal mental health. Key tasks involve working collaboratively with multidisciplinary teams, performing home visits, managing referrals, and facilitating safe discharges from the Andersen ward mother and baby unit. The position requires flexibility to meet community needs with working hours typically spanning Monday to Friday, from 9 am to 7 pm.
