The Midlands Partnership NHS Foundation Trust is seeking a Community Mental Health Practitioner to join their Children and Young People's (CYP) crisis and intensive support team in Lichfield. The ideal candidate will have a background in social work, mental health, children's, or learning disabilities nursing and will be responsible for delivering high-quality clinical services to children and their families. Key responsibilities include care coordination for an assigned caseload, delivering profession-specific mental health interventions per care plans, and managing a complex caseload under supervision. The role also involves conducting assessments, developing care programs, executing and evaluating treatment interventions, and implementing robust risk assessments and management plans.

Additionally, the practitioner will participate in service development and ensure clinical governance processes are followed within the team. The position demands adherence to professional codes of conduct and an ability to foster positive risk-taking to enhance the quality of life and independence of the young patients. Regular supervision and support from senior staff will be provided to help achieve the team’s objectives and align with the overall aims of the organisation.
