This post will be located in the dynamic and multi cultural city of Peterborough . We are seeking to recruit dynamic and enthusiastic Band 6 Care Coordinators: Nurses, Social Workers or Occupational Therapists. The practitioner will provide specialist advice, assess, plan and implement care to a designated caseload whilst working at all times to promote thesafety and well-being of service users and their families/carers.

The post holder will be an autonomous practitioner and responsible for a defined caseload of service users.
The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under pressure and with a desire for innovation.
We are able to support agile working for staff, using all appropriate I.T equipment and support.
o To undertake assessments of individuals and their families in the designated care group with complex health presentations, includingthose service users presenting with higher levels of risko They will work alongside the service user to develop their care plan along the recovery principles.o To offer specialist assessment and advice where required.o To assess, manage and regularly review on going risks as identified during the assessment, ensuring that this is done in collaborationwith the service user and carers.o To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.o To provide supervision of junior staff and trainees where appropriate (mentorship and preceptorship qualifications)o To act within the relevant Code of Conduct dependent on your Registration Body.
