The role of "Residential Placement Coordinator (Social Worker)" at the Cornwall Partnership NHS Foundation Trust in Bodmin is a community-based position requiring significant travel across the county. The successful candidate will serve as a Key Worker for service users receiving adult social care and/or ICB-funded support packages, ensuring the quality and integrity of these services. Responsibilities include conducting assessments, participating in multidisciplinary team meetings, attending mental health tribunals, creating risk management plans, and identifying suitable residential care options. The role also involves collaboration with various stakeholders and may require occasional overnight stays for out-of-county placements.

Additionally, the post holder will engage with families and carers to enhance support services, gather feedback through questionnaires, and facilitate involvement in meetings. The ideal candidate should demonstrate strong social work competencies and an ability to manage travel requirements effectively, contributing to the overall mission of delivering high-quality care within the Community Mental Health Service.
