The Low Clearance Specialist Nurse at Hull University Teaching Hospitals NHS Trust plays a crucial role within a multidisciplinary team, focusing on the care of patients with Advanced Kidney Disease (AKD) who are nearing the need for Renal Replacement Therapy (RRT). Responsibilities include providing specialist support, education, and advice to patients, their families, and health professionals regarding chronic kidney disease management, treatment options such as renal transplantation and dialysis, and lifestyle modifications. The nurse will also coordinate patient care, ensuring timely access to therapies and vaccinations, and will engage in shared decision-making processes to empower patients in their treatment choices.

Additionally, the post holder will foster effective communication within the care team, maintain up-to-date knowledge in the field, and contribute to the education of colleagues and community teams. This role emphasises collaboration, relationship-building with external networks, and ongoing professional development, while also aiming to achieve compliance with British Renal Association and NICE Standards for patient care.
