NHS Dorset is seeking a Senior Practitioner for their Personal Health Commissioning Team in Dorchester. The ideal candidate will be an experienced registered nurse (in Adult, Mental Health, or Learning Disability), Social Worker, or Allied Health Professional, with a background in Continuing Healthcare (CHC) or a desire to develop in this area. The role involves managing a caseload of CHC eligible individuals, conducting eligibility reviews, and assessing needs to recommend appropriate care options. The practitioner will work collaboratively with healthcare professionals, patients, families, and carers to ensure high-quality, person-centred support.

This position is predominantly desk-based but requires off-site assessments and meetings. Candidates should have strong analytical skills, a thorough understanding of relevant frameworks and acts, and must possess a full UK driving licence with access to a vehicle.
