The role of "Urgent Care Practitioner - Clinical Assessment Service" at Lincolnshire Community Health Service NHS Trust involves providing virtual assessments for patients through telephone and video consultations. The practitioner will utilize their clinical judgment to assess health needs, provide medication advice, and guide patients on appropriate levels of care, including at-home symptom management. This position is based in Beech House, Lincoln, and demands a flexible schedule, including shifts covering 24 hours a day year-round, with commitment to working 2.5 weekends per month.

Candidates will be expected to handle a variety of clinical presentations over the phone, from breathlessness to fever, while facilitating seamless patient pathways through different levels of care. Although experience in telephone assessments is not necessary, the successful applicant will receive comprehensive training. The role also includes providing clinical leadership and support to colleagues, ensuring a high standard of care and performance within this award-winning service.
