Prepare proactive care plans for vulnerable patients.

Provide peer support to patients, supporting their needs including navigation, signposting and direct support in enabling them to achieve their plans / goals.

You will be working as part of our new small mental health team delivering care and support to individuals with mental health needs in GP practices in Erewash.

You will be working collaboratively with the wider team which includes the voluntary sector, social prescribers and GP practices.

Interviews for this role will be held on Wednesday 13 November 2024.

As a Peer Support Worker, you will engage with individuals to show empathy, share experience where appropriate, inspire hope and promote recovery with the aim of assisting individuals to gain and maintain independence in the community.

Offer short interventions to support individuals with low level mental health needs e.g., anxiety, depression, stress etc.
