The Urgent Care Practitioner (UCR) in Whitchurch, working with Shropshire Community Health NHS Trust, will be an experienced Allied Health Professional or Nurse responsible for assessing and treating patients in urgent situations. As a senior clinician, this role involves identifying complex patient needs, designing and executing care programs under the guidance of Medical and Advanced Clinical Practitioner (ACP) leads, and ensuring safe clinical decision-making. The practitioner will collaborate with other healthcare professionals, provide leadership, mentor junior staff, and manage both acute and chronic conditions through a combination of medication and evidence-based practices.

Additionally, the role includes supporting a seamless transition between Urgent Care Response and Planned Care workstreams, implementing a see, treat, and review model to evaluate patient responses to interventions. Responsibilities also encompass diagnosing patient conditions, prescribing medications, and offering guidance on treatment plans in partnership with patients and their families.
