The Blackburn with East Lancashire Hospitals NHS Trust is seeking a Band 6 Occupational Therapist for a community rehabilitation role. The successful candidate will work within a dynamic community therapy service, providing rehabilitation to patients in their homes or current living situations after hospital discharge or for long-term condition management. This autonomous position involves assessing, intervening, and managing a diverse caseload of patients with various conditions, while also supervising other staff members and collaborating with multidisciplinary teams and external stakeholders.

The role requires the therapist to rotate across different therapy teams within the Community and Intermediate Care division, ensuring clinical and workforce development. Candidates must possess a valid driving license and access to a vehicle for work-related duties, as the service operates six days a week, including weekends and bank holidays on a rota basis.
