Midlands Partnership NHS Foundation Trust is seeking two Community Band 5 Staff Nurses to join their rapid response nursing team in Burton upon Trent. This role involves providing nursing care to patients in their homes as part of a 24/7 service, aimed at delivering high-quality, integrated care that helps patients maintain independence while managing acute health issues. The position is open to both newly qualified and experienced nurses who can assess, plan, provide, and evaluate evidence-based nursing care and work collaboratively with multi-professional teams to prevent hospital admissions and facilitate timely discharges.
