Malmesbury Community Team
37.5 Hours per week - Permanent
The Malmesbury and Royal Wootton Bassett Community Team is looking for an inspirational Specialist Community Nurse to lead a team in delivering high standard, quality care to patients within the community setting. Post holders will be SPQ qualified or willing to work towards this qualification. Wiltshire Health and Care is proactive in developing community specific training and can offer a supportive and friendly environment in which to learn new skills.
Our Community Nurses in Wiltshire Health and Care deliver care in patients homes and residential homes as part of a Multi-Disciplinary Team, linking closely with the GP Practices and other community services in the local areas. We work with the aim of preventing unnecessary admissions to hospital and in assisting people to manage their conditions in their own homes. The Malmesbury Community Team is split into three clusters (Malmesbury/Sherston, Purton/Cricklade and Royal Wootton Bassett) and we are a multidisciplinary team including Nurses, Therapists, Clinical Leads, the Home First Team, Care Coordinators and other staff.
The team is based in the Malmesbury Primary Care Centre and operates a shift pattern from 07.00 22.00. Nursing staff will be expected to work weekends and will be expected to cover early and late shifts., Evidence of up to date clinical professional development is essential and experience in front line NHS community patient care is preferable.
Applicants need to have excellent clinical skills, well developed communication skills and be able to work under pressure. The role is demanding but highly rewarding, and we are looking for an enthusiastic, motivated and hardworking individual who can work flexibly and efficiently.
