MENTAL HEALTH NHS NMC SOCIAL WORK
The Mental Health Practitioner role with NELFT in Kent and Medway focuses on providing mental health support to children and young people within Primary Care Networks (PCNs) across the region, including locations such as Tonbridge, Maidstone, and Aylesford. This position is part of a transformative initiative aimed at enhancing community mental health services by embedding mental health professionals—such as social workers, nurses, occupational therapists, and psychological therapists—directly in GP practices. Successful candidates will work within integrated teams to offer high-quality mental health care in primary settings, bridging the gap to secondary services and facilitating access to various interventions.

Candidates should possess strong assessment skills and a thorough understanding of local services, as they will serve as a crucial link for referrals to secondary mental health services. The role requires experienced practitioners who can work autonomously in a dynamic environment. Full and part-time positions are available, and prospective applicants will undergo a probationary period upon starting. This is an exciting opportunity for mental health professionals looking to make a meaningful impact in community settings.
