The Mid Furness Home Visiting Practitioner role at Morecambe Bay Primary Care Collaborative in Ulverston requires a motivated and organized professional to work 22.5 hours per week across three days. The practitioner will autonomously assess and treat housebound patients within the Primary Care Network (PCN), develop and implement treatment plans, arrange clinical investigations, and facilitate referrals to secondary care and community services. The position emphasises high-quality integrated care, continuous professional development, and collaboration with fellow practitioners to enhance service delivery.

Key responsibilities include advanced assessments, independent prescribing, maintaining clinical governance, mentoring colleagues, and staying current with healthcare guidelines and research. The practitioner will also engage in ongoing feedback and evaluation to uphold excellence in patient care and contribute to the overall improvement of home visiting services within the PCN. Candidates are encouraged to demonstrate their experience in primary care and ability to formulate accurate treatment plans autonomously.
