Southwell Medical Centre is seeking two Advanced Nurse Practitioners/Advanced Clinical Practitioners to provide high-quality primary health care within a multi-disciplinary team. The roles involve a total of 55.5 hours, with one position offering 30 hours over four days and the other 22.5 hours over three days. Key responsibilities include developing health programs tailored to community needs, managing acute and chronic medical conditions, performing patient consultations, and supporting patients with mental health concerns. The practitioners will also be expected to act as independent or supplementary prescribers, maintain accurate clinical records, and contribute to practice targets while ensuring comprehensive patient assessment and treatment.

The role demands advanced clinical skills to deliver care in line with clinical evidence, NICE guidelines, and NSF standards. Practitioners will be responsible for identifying at-risk patients, managing chronic disease treatment plans, and providing opportunistic health advice. Additionally, they will liaise with external services to support vulnerable patients and handle safeguarding matters according to national and local policies. House visits to care for housebound patients may also be required. The successful candidates should be friendly and conscientious, complementing the existing team's efforts in delivering effective health care to the patient population in Southwell.
