Working Full Time: 37.5 Hours Per Week
Due to the success of the ward-based Discharge Co-Ordinator pilot and the plan to expand the service within the Discharge Team we are seeking proactive and hands-on Discharge Co-Ordinators to join our dynamic team. The post holders will support in facilitating effective, efficient and timely discharge from hospital for our pathway 1-3 patients ensuring we maintain a home first approach.
The expansion of the team is an ideal opportunity to be involved of the transformation of the way Integrated Discharge processes functions and will enhance the partnership working with nurses, therapists, and external agencies.
In actively assisting and supporting patients and staff in various capacities of discharge planning and processes your dedication and enthusiasm will contribute to the smooth operations of hospital flow. You will be joining a team that is fast-paced, friendly and forward thinking.
If you have good communication skills, can engage with multiple professionals along with patients and their families to ensure discharge home is at the centre of planning then this is the opportunity for you.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £23 (standard) or £43 (enhanced), this cost will be deducted from your salary over the first three months of employment.
You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.
New entrants to the NHS will commence on the minimum of the scale stated above.
