4 days a week role
We are looking for an experienced individual to support service users requiring increased support from the Community Mental Health Team (CMHT). The team has a strong group of care co-ordinators, however there are times when service users require more input than a care co-ordinator can provide. At these times we utilise 'shared care'. We are looking for an individual to lead on the 'shared care' work. This is pre-planned work supporting service users who may have increased risk, emotional distress, or relapse in their mental illness. As these are the service users, we are most concerned about, there is an expectation of joint working with the Crisis Resolution Home Treatment and Mental Health Liaison Teams. As part of this role you may be asked to 'in reach' service users admitted to a ward or to complete 'soon' assessments for service users referred to our service.
Along with the shared care role there will be a need to complete new service user initial assessments to understand care needs and to plan care or signpost to the most appropriate service. As the first point of contact for our team we are looking for an approachable, professional and compassionate individual.
The Winchester Adult CMHT is looking for an enthusiastic, dedicated, and talented professional to join the team.
To fulfil the requirements of this role you need to hold a professional registration in Social Work, Mental Health Nursing or Occupational Therapy and be competent in completing high quality assessments of individual's needs. You may be required to cover clinics as necessary (qualification dependent).
To be an active member of the multi-disciplinary team supporting the assessment and care planning for service users.
To support patients with complex needs who are in a community crisis and require shared care.
Care co-ordination for patients with severe and enduring mental health difficulties.
To undertake ongoing assessment, treatment and evaluation for service users known to the service.
To support the delivery of therapeutic interventions.
Support the transformation of the team to deliver local evidence based care
