This is an exciting and unique opportunity to join the newly developed Forensic Intellectual and Neurodevelopmental Disabilities (FIND) Community Team in South London and Maudsley (SLaM).
The FIND Team will provide safe transition for inpatients discharged from the inpatient Forensic Services to the community providing specialist assessment, treatment and consultation to the Forensic Service in order to ensure a smooth, safe and clinically justified pathway to individuals with mental illness, offending history and learning disability with or without neurodevelopmental disorder.
The three forensic services of SLaM, South West London and St Georges( SWLSTG), and Oxleas together comprise the South London Mental Health and Community Partnership (SLP). The SLP's overarching goals centre on bringing care closer to home, improving patient pathways and outcomes by delivering excellent care in better environments. The SLP has a track record of delivering highly effective, clinically informed service developments in order to better meet the needs of service-users. The FIND provision is one such development and the SLaM FIND Team will therefore work closely with the FIND Teams in each of the other Trusts in order to expedite service-user's transition through inpatient settings to community provision.
Work in partnership with MDT colleagues to support a case load of service users with complex needs, using evidence based and service user centred principles
Provide specialist Occupational Therapy for a caseload of service users, completing assessments, interventions and outcome measures underpinned by Model of Human Occupational and the Recovery Approach
Participate in the planning, development and evaluation of services provided
Provide placement education opportunities and supervision for OT students on practice placements and junior members of staff as required
Carry out multidisciplinary activities where appropraite e.g. involvement in team quality improvement projects
To work collaboratively and effectively within a multi-disciplinary community team liaising with inpatient services and community-based services as required
To support service users to identify recovery goals, develop daily living skills and improve motivation and confidence in order to live as independently as possible
To support service users with any physical health or physical disability needs whilst interfacing with local services to ensure adaptive equipment is sought and fitted as necessary
1. Select and apply OT assessments underpinned by MOHO Theory, for a designated caseload to support case formulation and intervention planning.
2. To work with service users to identify OT goals as part of the overall care plan, working within the CPA process.
3. To plan and implement individual and/or group interventions, in collaboration with the service user, using graded activity to achieve therapeutic goals.
5. To monitor, evaluate and modify treatment for service users with multiple needs in order to measure progress and ensure effectiveness of intervention.
6. To consider both physical and mental health needs as part of the occupational therapy process
7. Assess the occupational and vocational needs of the service user group signposting and/ or supporting access to community and employment opportunities where appropriate
8. Provide clinical training and supervision to service users extended support network as appropriate.
