The role of Acute GP in the Medical Same Day Emergency Care (SDEC) Unit at University Hospitals Plymouth NHS Trust involves providing urgent medical care while facilitating patient transitions between hospital and community-based services. The position entails working alongside acute physicians to enhance patient access to urgent care, manage referrals, and support the Integrated Virtual Ward, which aims to deliver high-quality care at home, reducing hospital admissions and enhancing patient well-being. The GP will engage in initial assessments, diagnostics, and collaborate with a multidisciplinary team to ensure comprehensive care planning and discharge processes.
