The Medicines Optimisation Technician at Calderdale and Huddersfield NHS Foundation Trust will play a crucial role within the Community Healthcare Division, delivering clinical pharmacy services to patients in their homes. This position supports the Virtual Ward and Urgent Community Response Team by ensuring safe and effective medication use for adults who are registered with GP practices in Calderdale and Huddersfield. The technician will conduct medicine reviews, provide essential information and advice to patients and healthcare professionals, and coordinate services to ensure adherence to medicines optimisation protocols.

Key responsibilities include performing medication audits, training less experienced technicians, managing sensitive patient information, and maintaining up-to-date patient medication records using relevant electronic systems. The role also involves collaboration with multidisciplinary team members and other health and social care organisations to improve patient care and support cost efficiencies in prescribing. The technician must maintain professional registration as a Pharmacy Technician and engage in continuous professional development while advocating for both patients and colleagues.
