The Community Matron role at West London NHS Trust in Southall involves leading and managing the Southall Locality Team to deliver high-quality, person-centred care to patients. The position includes overseeing clinical and management support for team members, ensuring effective care for individuals with complex needs, and fostering collaboration with multidisciplinary teams and external providers. The matron will be actively involved in developing personalized care plans to prevent unnecessary hospital admissions and promote timely discharges while also empowering patients and their families in self-care practices.
