Linthorpe Surgery in Middlesbrough is seeking a Care Coordinator to support childhood immunisations and vaccinations for their Primary Care Network (PCN) with a patient population of 45,000. This part-time role (24 hours per week) involves administrative and patient support tasks, including booking appointments, managing IT systems, coordinating clinics, and record-keeping. The ideal candidate should possess strong organisational and patient care skills, with an understanding of patient needs. While previous healthcare experience is beneficial, it is not mandatory, and training will be provided.

Key responsibilities include using GP systems to identify patients needing vaccinations, coordinating immunisation clinics across three practices, monitoring appointment systems, and collaborating with administrative teams. The Care Coordinator will also adhere to practice protocols, escalate any safeguarding concerns, and participate in training events as required by the PCN Management Team.
