This is an exciting opportunity for an experienced nurse to join our Teenage and Young Adult Cancer Service.

The TYA Service is a regional service that provides holistic and expert care to young people aged 16-25 with cancer under the care of the Principal Treatment Centre in Leeds Teaching Hospitals Trust, and covering the Yorkshire and Humber TYA Clinical Delivery Network (CDN)

The successful applicant will have experience in either childrens and/or adult cancer nursing. You will require excellent communication and interpersonal skills and an ability to work both autonomously and as part of the wider MDT.

Work closely with the TYA Multidisciplinary Team as part of the TYA Service.

Acting as a Key Worker to provide expert support, information and advice to young people with cancer aged 16 - 24 years and their families

You will assess their care needs and plan, implement and evaluate programs of care for the young person and their family.

Working closely with the TYA team to enhance care by promoting patient choice using age appropriate skills and TYA expertise

Work with the TYA service to provide equitable access to supportive care across the Yorkshire and Humberside Region

Work with Designated hospital teams to provide age appropriate care to young people aged 18-24

As a core member of the TYA MDT, contribute to the multidisciplinary discussion and patient assessment and core planning decisions

Provide education, advice and support to health care professionals on the specific needs of Young people with cancer.

Will liaise with other agencies in health, social care, education and charity partners to provide a comprehensive age appropriate package of care to support these young people.

The Jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care.

The jobholder will provide specialist nursing care, treatment and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources.

The job holder will support the development of interdisciplinary and whole service working. As a role model they will promote the sharing of good practice and delivery of high quality, effective and efficient care. The job holder will provide training and education to staff groups, patients and families/carers.
