Medway Community Healthcare is seeking adaptable and collaborative Community Nurses to join their neighbourhood nursing service in Chatham. The role involves delivering personalized nursing care to housebound patients aged 18 and over within the Medway area, supporting individuals with complex and long-term conditions to maintain their independence at home. The service operates every day of the year, providing care that aligns with Buurtzorg principles, emphasizing self-care, patient engagement, and integrated support in collaboration with other health and social care professionals.

Candidates will benefit from a comprehensive induction program, ongoing clinical supervision, and opportunities for professional development, while contributing to a rewarding work environment focused on patient satisfaction and team collaboration.
