Somerset Foundation Trust is the first NHS Trust on the English mainland to take a joined-up approach to providing clinical, acute hospital, community health, mental health, learning disability and dental care services.

Due to retirement of the current post-holder, a rare opportunity has arisen within Burnham and North Sedgemoor neighbourhood of Somerset NHS Foundation Trust, for a suitably qualified and experienced pharmacy technician to provide pharmacy support in the Community Rehabilitation Service (CRS).

The CRS focus is working with patients in their own communities, helping them to regain any lost skills, increase their levels of independence and wellbeing and to empower and equip them to make lasting changes for their future. CRS provide support and guidance to help patients to achieve their goals by encouraging them to contribute to decisions about their treatment.

Pharmacy Technicians have been successfully supporting patients to manage their medicines in their own homes in Somerset as part of their multidisciplinary teams for many years. Due to the retirement of the current post-holder we are now seeking a Full Time (37.5 hours per week) Pharmacy Technician for the Burnham and Bridgwater area of Somerset.

You will possess a good knowledge of medicines optimisation which you will have already applied in a Community / Retail / Primary Care / Hospital / Prison or ICB setting.

A good team player who is self-motivated, resourceful, enthusiastic and with excellent communication skills, you should be confident and capable of working on your own as these posts involve a significant amount of lone working including in patient's own homes. Due to the nature of this post a full driving licence and own form of transport is essential.

You must be registered with the General Pharmaceutical Council and have a locally or nationally recognised Medicines Management qualification.

The role will include post-hospital discharge medicines reconciliation, assessing and supporting patients' compliance with their medication. The role will involve close contact, including domiciliary visits, with patients throughout the area and liaising with a number of different healthcare professionals including those in the patient's GP practice, Primary Care Networks, hospital, and community pharmacies.
