                                                    Care Coordinator  with Conexus Healthcare CIC in Normanton, West Yorkshire

                                                     Care Co-ordinators provide extra time and expertise to support patients in preparing for, or in following-up, clinical conversations they have with primary care professionals. They will work closely with the GPs and other primary care professionals within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed.  The post holder will support the running of the care home and housebound team, supporting the development of care plans, co-ordinating patient care services, and working with the care team to evaluate interventions. You will need to display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service. The role would be ideal for someone with experience in primary care. You would be joining the care home and housebound team made up of ANPs, Pharmacists and Nurses. The team has established a strong reputation after starting 18 months ago. The team are looking forward to developing their roles and seeing their team grow. The Care Coordinators have become an asset to the PCN and the practices/patients that they serve. Please note this is not an Agenda for Change Contract, however we are able to offer the NHS Pension Scheme to the successful candidate(s).     The role of the Care Co-ordinator is to proactively:  Identify patients who are deemed as high users and support with non-medical interventions and referrals to external partners.   To support the multi-disciplinary team (MDT) meetings and the ongoing patient case management.  To liaise with the additional roles staff in the PCN to meet unmet patient needs.  Support patients to retain their independence.  Engage with patients living in care homes.  Support practices with housebound patients.   Act as a link between external agencies and General Practice.
