Care Co-ordinators provide extra time and expertise to support patients in preparing for, or in following-up, clinical conversations they have with primary care professionals. They will work closely with the GPs and other primary care professionals within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed.
The post holder will support the running of the care home and housebound team, supporting the development of care plans, co-ordinating patient care services, and working with the care team to evaluate interventions. You will need to display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service. The role would be ideal for someone with experience in primary care.
You would be joining the care home and housebound team made up of ANPs, Pharmacists and Nurses. The team has established a strong reputation after starting 18 months ago. The team are looking forward to developing their roles and seeing their team grow. The Care Coordinators have become an asset to the PCN and the practices/patients that they serve.
