The Cornwall Partnership NHS Foundation Trust is seeking motivated Healthcare Assistants for the Coastal Community Nurse Team in Blackwater, offering the chance to assist in providing nursing care to adults in their homes, particularly those nearing end-of-life. Successful candidates will carry out a range of nursing duties, such as wound care, venepuncture, catheter care, and pressure area management, while working as part of a community nursing team within a flexible shift system that operates from 8 am to 10 pm. Applicants must possess previous care experience, a full UK driving license, and the ability to independently travel within the designated area.

The role emphasises integrated care to achieve optimal patient outcomes and involves tasks that require autonomy, ensuring high-quality care delivery while supporting registered nurses. The Community Nursing Service in Central Cornwall is offering a fantastic opportunity for highly motivated and enthusiastic Community Health Care Assistants to join our service. The successful candidates must be able to demonstrate a passion for delivering nursing care to adults in their own homes, including those who are approaching end-of-life. The role will involve carrying out planned nursing care which may include wound care, venepuncture, catheter care, and pressure area care.

We believe in integrated ways of working to achieve the best possible patient outcomes. Previous care experience is essential. The trust is committed to personal learning and development within Community Nursing with many opportunities available.

Due to the nature of the role it is essential that applicants have a full UK driving licence, and are able to travel independently on a daily basis to and from the base, patients' homes and other venues around the geographical area. Mileage is paid at agenda for change rates and travel time between visits is incorporated into your working shift.

There are opportunities available across Central Cornwall including Falmouth, Truro and the Coastal (Perranporth & Chacewater) teams. The service covers from 8am to 10pm, consisting of 7.5 hour shifts.

To carry out assigned tasks while regularly working without direct supervision involving direct care to patients and carers to support the registered nurse.

Working as a member of the community nursing team within a given geographical area. The District Nursing Team work on a flexible rota shift system covering the period from hours which includes an on call system offering a high standard of service.

As part of a team, provide high quality care to patients, promoting recovery in a manner that maintains safety for all. The post holder will assist in service and good practice developments.
