An exciting opportunity has arisen for four Band 6 Practitioner to join the Mental Health Acute Response Service. This role involves supporting the ongoing development of the crisis pathway.
We are looking for dynamic, proactive, enthusiastic and highly skilled professionals with a minimum of 2 years post qualification experience who will form key part of the Integrated Crisis Acute Hub. The post holders will also play a vital roles in ensuring that all matters related to client's journey are seamless and of a high standard. This will entail working closely with all professionals and carers involved in client's care.Applicants will need to be experienced in Crisis intervention and have clinical experience in mental health, where a degree of autonomous working is required. They should be able to demonstrate the ability to work on their own initiative, have good time management skills and be able to prioritize workload effectively. Applicants will need to demonstrate well developed leadership and interpersonal skills especially when handling conflict and crisis, and should have the appropriate skills to demonstrate a good risk management.The service operates 24hr / week, 7 days / week and has very close links with the inpatient wards, Crisis Teams and Community teams ensuring that NELFT provides an accessible service for clients in need of mental health support within the borough of Barking, Havering, Redbridge and Waltham Forest.
Successful post holders will work alongside the following crisis team, namely Street Triage, Mental Health Direct, Liaison & Diversion service, EDT, Bleep Holders and Emotion Wellbeing out of hours service.
- Providing telephone triaging and supporting service users who are in
- crisis by carrying out assessments
- Making clinical decision in conjunction with the ICAH Clinical Lead to carry out the gate keeping function of the trust i.e. be able to establish whether admission to an inpatient setting is required or accept for Home Treatment Team input.
- Post Holder will be required to carry out an effective assessment and have the ability/ knowledge to signpost to appropriate organisation/ teams which will include face to face assessment in the community
- Work closely with inpatient services, Crisis teams and community teams to whole systems working for all our client
- Participating and making sound clinical decisions pertaining to bed management issues, Extra Contractual Referrals & Gate keeping role
- Post holder will also be expected to work across the other Response Team to ensure the Response service operates as a single point of access
About us
- Post Holder will work within the Acute Response Team to provide telephone triaging and support to service users who are in crisis; this will also include police officers dealing with service users in a public area.
- Providing counselling support to service user by encouraging clients to express their feelings and discuss what is happening in their lives.
- Guide Service users in the development of skill/strategies for dealing with their problems, ensuring crisis interventions plans are developed with service users and followed accordingly
- Counselling family members/carers to assist them in understanding, dealing with, or supporting service users.
- Collecting information about service users through telephone/face to face interviews to assess risk and adapting management plans to contain any risk posed or expressed.
- Evaluate service users physical or mental condition, based on review of client information.
- Signpost service users or family members/carers/police officers to community resources or other organisation if appropriate
- The role will also entails supporting police officers when making decision about service user in a public place pertaining to section S136.
- Attend face to face assessments in public areas/home environment when called out by police for support in relation to mental health crisis.
- Assisting police officer in managing people with Mental Health problems and working collaboratively with the police officers.
