We are looking for an enthusiastic and highly motivated band 5 Community Nurse to join the Dalton District nursing team. The successful applicant will be committed to working closely with health and social care colleagues to provide high quality care to patients with a wide range of medical conditions in the community setting.
The role requires excellent communication skills as you will be undertaking complex nursing assessments and developing and delivering programs of care for patients and carers. The district nurses work a 24hr service and regular weekends, evening shifts and occasional nights will be part of your rota.
You will need to work collaboratively with other professionals, voluntary sector and informal carers to promote excellent care to patients in their own homes and community setting.
You will be joining an enthusiastic and motivated team who are friendly and approachable. We are very patient focused and staff development is high on our agenda. As part of our team we want to develop all staff to the highest standard.

The post holder will work as a registered Nurse within Community Services, accountable for providing evidence based, holistic nursing care of patients.
The service covers a 24 hour period 235 days a year. It is the expectation of this post that the post holder will work weekends, bank holidays, evenings and night shifts as required.
The post holder will be required to work as part of the integrated team across community settings such as patients' homes, residential and nursing homes. Flexible working will be required to meet the needs of the patients group and the service as it continues to develop. Whilst working with other health care professionals the post holder will develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.
You will be expected to support the Sister/Team Leader in leading the team supervising, directing staff and delegating workload according to skills of the staff within the team.
