Longfield Medical Centre in Maldon is seeking a part-time Healthcare Navigator to join their team for 26 hours per week. The role involves flexible scheduling, including coverage during surgery opening hours and potentially filling in for holiday or sickness absences. As the first point of contact for visitors, the Healthcare Navigator will uphold the practice's mission to deliver high-quality primary care services, manage patient inquiries, handle appointment bookings, and process non-clinical requests related to prescriptions.
Key responsibilities include answering phone calls, managing online triage requests, filing records, and documenting patient information using the SystmOne system. The ideal candidate should be prepared to attend staff meetings and provide patient support when needed. The shift schedule includes morning and evening hours, with specific days off, and an emphasis on teamwork and adaptability.

Longfield Medical Centre in Maldon is looking for a professional healthcare navigator to join our growing team. You must be able to work flexibly in a team on a rota basis to cover surgery opening hours and also provide holiday and sickness cover, as needed, on an overtime basis.
The role is available on a part time basis for 26 hours per week, with the shift pattern below:
- Monday: 8:00 - 13:30
- Tuesday: off
- Wednesday: 8:00 - 13:30
- Thursday: 8:00 - 13:30
- Friday: 8:00 - 18:30 (includes 1-hour unpaid lunch break)
You will be the first point of contact for all visitors to the practice and will be expected to follow the practice mission statement of always aiming to provide high quality primary care services.
- Answering the telephone.
- Managing the Patch's (online triage) request.
- Dealing with all enquiries from patients attending the surgery.
- Booking in patients arriving for appointments.
- Dealing with non-clinical enquiries regarding prescriptions.
- Filing of notes, correspondence, results etc.
- Taking results via the telephone from hospitals.
- Attending staff practice meetings.
- Chaperoning for patients when necessary.
- Using SystmOne to send internal tasks, access patient records, send internal and external messages, write patient correspondence.
