The Manchester Macmillan Palliative Support Service is seeking an Assistant Practitioner/Nurse Associate to join their Specialist Palliative Care Team. This role involves providing comprehensive support for patients with life-limiting conditions, including those in care homes in Central Manchester. The successful candidate will work collaboratively within a multi-disciplinary team, which includes medics, dieticians, occupational therapists, physiotherapists, nurses, and speech and language therapists, to implement and evaluate rehabilitation and education programs aimed at enhancing patients' independence and quality of life.

The post holder will manage a small caseload under the supervision of a responsible clinician, focusing on maximizing health outcomes and enabling patients to receive care in their preferred settings.
