The Kent Community Health NHS Foundation Trust is seeking a Band 5 Community Public Health Nurse (CPHN) to join their Health Visiting team in Folkestone, focusing on children aged 0-5 years. This role offers both full and part-time opportunities, ranging from 22.5 to 37.5 hours per week, primarily in community settings and client homes. Responsibilities include supporting the Healthy Child Programme, conducting universal and targeted assessments, and delivering care packages based on evidence-based practice. The CPHN will work under the supervision of a qualified Health Visitor to promote public health, support healthy lifestyles, and address health inequalities through various interventions.
