More detail about the role: In the Community Matron role the post holder will be expected to have studied at a minimum of Degree level and be able to demonstrate long term conditions assessment and management skills and hold the V300 prescribing qualification or the Specialist Nurse Practitioner qualification. The expectation of the role is to ensure that patients receive appropriate care and treatment as close to home as possible and that Health and Social Care is patient centred and delivered to a high standard. This involves making close links with secondary care providers to facilitate discharge of patients as appropriate through the system. The post holder will be expected to develop professional relationships with Medical colleagues, other Health Professionals, Social Services, Voluntary Agencies, Secondary Care Hospitals, Community Hospitals and liaison staff, to enhance the Service and Care pathway provided to patients who have complex needs and their families. The post holder will be expected to independently explore all avenues to support patients with complex needs and chronic disease including forming effective communication networks across organisations to achieve patient focussed goals. To support the organisation the post-holder will participate in the Senior Clinician on-call rota and will be remunerated in line with the Trust's Single On-call Remuneration Agreement. The post holder will support and develop students of Nursing through mentorship. The Community Matron will be based in the Community setting with the St Thomas Community Nursing Team and work alongside the team providing clinical leadership to both staff and students. The post holder will fulfil all tasks and work as part of a team. To meet the needs of the service, the post holder may be required to work in other areas as appropriate and under the direction of the line manager or appropriate manager. The successful candidate would be able to demonstrate knowledge and understanding of current issues relating Primary Care, Community Nursing, Social Care and integration at interview and will be asked to present a short presentation. Please note that if there are no suitable candidates shortlisted for the Band 7 Community Matron role we may consider a developmental appointment at Band 6, Agenda for change pay scale. Once full development, evidenced by academic and clinical qualifications in the essential criteria for the role, has been achieved the successful candidate would progress to Band 7 Agenda for Change salary commencing on the lowest Band 7 salary scale . You will need to own and drive your own car as travelling between patient visits and across other venues is part of this role. There is a mileage allowance which is calculated to reimburse your expenses. Working Pattern: 30 hours per week. 8:30 to 17:00 are the core hours for the Community Nursing Service.
