The Community Matron position with the Royal Devon University Healthcare NHS Foundation Trust in Exeter is an opportunity to be part of a progressive healthcare team dedicated to delivering innovative home care and supporting patients with complex needs. The role involves managing long-term conditions, facilitating patient discharges from hospitals, and fostering collaboration within a multidisciplinary Health and Social Care team. Candidates are expected to have at least a degree, hold a V300 prescribing qualification or a Specialist Nurse Practitioner qualification, and possess strong assessment and management skills.

The Community Matron will work primarily in community settings, providing clinical leadership, mentoring nursing students, and ensuring high standards of patient-centered care. The role requires the ability to establish effective communication networks with various stakeholders, including primary and secondary care providers, social services, and voluntary organisations. In addition to standard hours, the role includes participation in an on-call rota and may involve travel for patient visits, for which a vehicle is necessary.
