We have an exciting opportunity for a dynamic Physiotherapists who is passionate about making a difference in proactive healthcare to join our Admission Avoidance and Discharge Service. We are seeking for a skilled Physiotherapists to lead in driving a proactive prevention approach and be at the forefront of proactive healthcare in a multidisciplinary team that will shape the future of Intermediate Care Pathways.
This new role is developed to take a lead role in collaborating with Care Homes, Reablement Service including Initial Assessment Team in Adult Social Care, Homeless Project in Foot Health Service, and ACP Physiotherapists in Community Frailty.
The post holder will lead on developing strategies that promote health, well-being, and independence, helping patients to stay out of hospital and support the delivery of holistic, patient centred care to vulnerable populations, including those with complex needs.

The post holder will provide a highly specialist source of expertise in clinical management and act as a one of the lead physiotherapist in AADS in order to prevent unnecessary admission to hospital. The post holder will ensure the quality and effectiveness of physiotherapy delivery within the service on a day to day basis, developing the service under the management and with the strategic direction of the clinical lead and therapy manager.
The post holder work alongside senior clinicians in the team to deliver a case management approach, for integrating, coordinating and pro-actively assessing patients' health and social care needs, so that the individual receives the best possible care in the most appropriate place. The role involves developing care plans to facilitate safe, timely and effective discharge, optimising patient outcomes and ensuring patients, carers and professionals have the skills and knowledge to implement those plans.
The post holder will be a member of the wider AADS service providing a range of interventions - from rapid response, falls pick up, admission avoidance in emergency department at the Royal London Hospital, intermediate care including falls prevention through to preventative action and support for self-management of long term conditions including joint clinic at homeless clinics in Foot Health Service. The post holder may be required to cover across the service when required e.g. Admission Avoidance in the ED.

The post holder will be required to work with partners across the health and social care interface to ensure a joined up and integrated approach is adopted to the provision of services. All staff are expected to work across traditional boundaries of both community and acute services, to deliver truly holistic care.
To be professionally accountable for all aspects of own work, including management of highly complex patients in your care to prevent relapses/injury and identify risks for admission. To ensure a high standard of clinical care for patients under your management, and support more junior staff to do likewise.
To provide rapid response, falls pick up, and specialist multidisciplinary triage in the community including physiotherapy input in Adult Social Care and Homeless Foot Health Service.
To formulate accurate physiotherapy assessment and prognosis of patients/clients presenting with a broad range of highly complex physical, cognitive and psychological conditions and disabilities by utilising interpreting and analytical skills in order to recommend the best client-centred treatment plan and develop comprehensive discharge plans in order to meet patients complex physical, cognitive, social and environmental needs.
To accept clinical responsibility for a designated caseload of patients and organise this effectively and efficiently with regards to clinical priorities and use of time. To ensure the junior staff do likewise.
To deputise for the Clinical Lead taking responsibility for operationally managing allocated parts of the service.
