The role of "Senior Mental Health Practitioner - Under 18's Community" with Norfolk & Suffolk Foundation NHS Trust offers an opportunity to engage with children and young people to provide intense community-based support, aiming to prevent inpatient admissions. The successful candidate will be part of the Child and Adolescent Transformation Team (CATAT), focused on empowering individuals under 18 through preventative care models, conducting enhanced assessments, and developing tailored care pathways in collaboration with children, families, and other professionals. Responsibilities include performing risk assessments, delivering therapeutic interventions, and contributing to successful discharge planning.

The position emphasises a systemic approach to mental health and integration with core community teams and partner agencies. The practitioner will travel across Norfolk and Suffolk, participating in multidisciplinary team meetings, providing training, and ensuring the least restrictive care environments.
