This dynamic position focuses on Medicine Management Optimisation, working within both General Practice and Care Homes. As part of our enthusiastic multi-disciplinary team, you will rotate between GP practices and the Care Homes Team, gaining invaluable experience and refining your clinical skills. Your expertise will be instrumental in improving medicines management and implementing effective strategies under the new Network Contract, ensuring our patients receive the highest quality care. This training role includes the opportunity to complete a CPPE course, allowing you to grow in confidence and capability while making a meaningful impact on the health outcomes of our community.
