The Birmingham and Solihull Mental Health NHS Foundation Trust is seeking a Band 6 Community Psychiatric Nurse (CPN) for their Older Adult Mental Health Community services within the West Community Hub. The successful candidate will be responsible for care coordination of a caseload involving patients with complex mental health conditions, requiring a strong understanding of the Care Programme Approach (CPA) framework. The role involves collaboration within a multi-disciplinary team and necessitates prior experience in Older Adult Services.

The ideal applicant should possess excellent communication skills to effectively engage with service users and their families, particularly those who are difficult to reach, and demonstrate the ability to work independently while managing workloads under pressure. Flexibility and a positive, proactive attitude are essential, as the post holder may also need to cover other roles and contribute to team initiatives during periods of absence.
