FAMILY MULTI-DISCIPLINARY NHS SUDDEN CARDIAC DEATH
The Patient Pathway Coordinator role at Guy's and St Thomas' NHS Foundation Trust involves supporting the Inherited Cardiac Conditions (ICC) team within the Cardiology Service for an 18-month fixed term, with potential for extension. The position requires collaboration with various clinical and managerial staff to facilitate the implementation of new initiatives, such as the sudden cardiac death program and the Phenotips system. The coordinator will play a critical role in delivering effective support to families affected by sudden cardiac events and will provide administrative backing to consultants, nurses, and genetic counseling teams. Key responsibilities include managing clinic schedules, organizing multi-disciplinary meetings, and ensuring patient care complies with access policies.

The successful candidate will work autonomously under the guidance of the Assistant Service Manager for Cardiology, position work both on-site and remotely at King's College Hospital, and will be an integral part of enhancing service delivery and patient experience within the ICC.
