The Countess of Chester Hospital NHS Foundation Trust is seeking a dedicated and compassionate Assistant Practitioner/Nursery Nurse to work full-time (37.5 hours per week) within a community team that provides specialized care to children and young people with complex health needs. The role involves collaborating with nursing and medical staff, family members, and multi-professional teams to deliver individualized clinical interventions in various settings, including home, school, and hospital environments. The position requires flexible working across day, evening, and night shifts, including weekends and Bank Holidays, as part of a service that operates 24/7.
