All Age Continuing Care delivers the patient specific Learning Disabilities and Autism Programme Integrated Care Board responsibilities.
In some areas positions will be within an integrated role with duties across Integrated Commissioning or Local Authorities.
This is a challenging post which will require dynamic leadership and excellent communication, risk mitigation, data analytic and financial management skills along with a proven capacity to work across the multidisciplinary spectrum to engage key stakeholders to deliver the NHS All Age Continuing Care agenda.
This is a key tactical post straddling the strategic/operational interface within NHS All Age Continuing Care services.
Extensive understanding of the national perspective and future strategy for the NHS All Age Continuing Care service and related to areas of health and social care, ensuring that staff are fully aware of the implications and can contribute effectively to service improvement.
Contribute to strategic workforce planning, governance, quality improvement, financial and performance management processes and will have responsibility for the effective implementation of these processes across those services for which they have responsibility.
Responsible for ensuring that NHS All Age Continuing Care is delivered in accordance with relevant statute and guidance, including (but not limited to) the NHS Standing Rules, the National Framework for NHS Continuing Health Care and NHS-funded Nursing Care.
Accountable for ensuring that all staff have the required skills, knowledge and competence regarding policy, processes and guidance around NHS Continuing Healthcare, Funded Nursing Care, Fast Track and All Age Continuing Care within their service.
Accountable for ensuring that the service is delivered with the service user at the heart of the process and that interventions are completed in line with best practice and outcomes.
Accountable for ensuring the active management of the clinical caseload and numbers of cases awaiting assessment, review or case management, supporting reporting mechanisms to ensure Hampshire and Isle of Wight Integrated Care Board (HIOWICB) is kept updated as part of performance assurance of NHS Continuing Healthcare.
Delivery of Cost Improvement Programmes.
To have specialist knowledge on NHS All Age Continuing Care and associated legislation. To provide advice and guidance and where necessary advising the ICB on interpretation but gain appropriate legal advice to ensure that the organisation is in line with changing legislation.
