The Derbyshire Healthcare NHS Foundation Trust is seeking a Mental Health Practitioner (RMN, OT, or Social Worker) to join their Living Well Community Mental Health team in Swadlincote. This role involves conducting initial assessments to help individuals access support and work towards recovery goals. The practitioner will manage a caseload of clients with varying mental health challenges, collaborate with a multi-agency team, and maintain a person-centered focus.

Candidates should possess strong communication skills, a comprehensive understanding of health and social care legislation, and experience in risk assessment and management. The position offers full induction and ongoing training, as well as opportunities to mentor and supervise other staff and students. This is a fixed-term position for maternity cover, and applicants must be capable of transporting themselves and service users across a geographical area.
