The post holder will be responsible for the comprehensive treatment & assessment of adults experiencing problems with occupational performance impacting on function resulting from frailty syndromes. Together with the individual they will assess needs, agree goals, implement intervention plans and evaluate progress. The role will support both the GP practice-based staff and members of the Neighbourhood Team to identify and support people to reduce the risk of unplanned hospital admissions and to effectively support those individuals in the community.
To independently support a defined frailty caseload with indirect supervision from the Primary Care Networks Neighbourhood Team Lead.
The post holder will provide specific occupational therapy interventions and professional advice and support to other members of the multi-disciplinary team.

This is a community-based post providing initial & on-going assessment and support to people referred through Primary Care and Neighbourhood Team practitioners. The post will sit within the Grantham & Sleaford Neighbourhood Team, supporting local populations in managing challenges experienced from physical and mental health conditions.
