We are currently looking for a Practice Nurse to join our team here at Grove House Surgery in Bognor Regis.

The successful applicant will be involved in several clinical areas such as chronic disease management (diabetes in particular but not essential), vaccination and immunisation, health promotion and all other primary care services.

Job responsibilities

Our nursing team offers a range of clinical services including health promotion and long-term condition management, NHS Health Checks, hypertension management, contraception and sexual health advice, cervical screening, wound management, travel advice, administration of national childhood and adult vaccinations and phlebotomy.

To maintain robust systems of data collection, audit and review of all clinical activities, protocols, care pathways and clinical systems, as needed to ensure good patient care and service is maintained.

To participate in Infection Control procedures and protocols and their implementation within the practice.

To support in all practice services including Key Performance Indicators, Locally Commissioned Services, Quality Outcome Framework and other incentive schemes.

POST: Practice Nurse

SALARY: £38,335 £39,106 dependant on experience

BASE: Grove House & Arthur Griffiths Branch Surgery

ACCOUNTABLE TO: Lead Nurse/Gps/Practice Manager

HOURS: 25 - 30 hours per week - hours could be flexible

CONTRACT: Permanent


General:
To provide nursing care as needed for patients. BP, urinalysis, phlebotomy, microbiology samples, ECG, and other practice based tests as needed in the course of the care of patients.To work with the doctors and nurses to support and care for patients through assessment, diagnosis and management of their health needs.


Chronic Disease Management:
To provide management of chronic diseases such as Diabetes, COPD, Asthma including maintaining registers and constructing care pathways as appropriate.


Immunisations:
Providing childhood immunisations and travel vaccinations.To support the annual flu campaign and other mass population vaccination programmes as needed in line with national guidance.


Wound care
:To provide care for minor injuries, wound care (leg ulcers), including the management of compression bandaging and to work to create safe management plans as needed.To liaise directly, where appropriate with community and secondary care, tissue viability teams to support the care needs of the patient.To provide supervision for HCAs undertaking wound care in the practice.

Quality Improvement and Clinical Audits:To establish and participate in a robust system of clinical audit in partnership with other clinical and administrative colleagues leading to measurable quality improvement activities.

Administrative:To maintain accurate patient records using Systm1, completing all documentation, computerised and paper-based, as is required for good patient care.
