The role involves being a clinical pharmacist at our practice with a patient list size of around 18,000 patients.As a member of the general practice clinical pharmacy team you will be expected to deliver a high quality clinical pharmacy service to patients ensuring safe, clinically effective and cost efficient use of drugs aswell as using expertise in independent prescribing to support better access to medicine.
Your role will be focused on general practice support, assessment of patients with long term conditionsand ability to independently prescribe, medicines optimisation and reviews, aligning repeat prescription, acting on hospital letters and discharge letters, meeting local/national prescribing targets, working to the practice requirement anddealing with minor illnesses.We expectour clinical pharmacy team to evaluate the delivery, implementation and risks involved with our services as well as contribute to clinical governance agendas and regularly review policies and procedures with ideas to develop these in a multidisciplinary manner.
Your main duties include but are not limited to:- Medication reviews and queries including long term conditions- Audits for safe prescribing- high risk drugs monitoring- Practice CQC and MHRA Guidance compliance- Reconciling of practice purchased/administered drug-Completion/action ofDocman/ tasks sent to pharmacist- MMT requirements and targets- Supervisingpharmacy team admin staff and pharmacy technicians (if employed)
