The Community Psychiatric Nurse in the Crisis Function at Hertfordshire Partnership University NHS Foundation Trust in Stevenage is responsible for assessing care needs, developing and implementing care programs, and managing a defined caseload of service users. This role includes working closely with a multi-disciplinary team and ensuring that care plans are effectively executed while maintaining high standards of record-keeping and compliance with Trust policies. The nurse will also conduct risk assessments, provide ongoing interventions, and foster positive relationships with service users, their families, and other agencies, charities, and providers to promote independent living within the community.

Additionally, the post holder will supervise and support the development of staff and students, contribute to service philosophy and strategy discussions, and demonstrate knowledge of legal and ethical issues pertaining to mental health care. Effective collaboration with other teams, such as Crisis Teams and Intensive Outreach Teams, is essential, as well as adherence to the Trust's commitments to equality, diversity, and anti-discrimination.
