The Townships 1 Primary Care Network in Sheffield is looking for a dedicated Pharmacy Technician to join their frailty team. In this role, the Pharmacy Technician will collaborate with GPs, nurses, and other healthcare professionals to conduct medication reconciliation for patients recently discharged from hospitals and to review long-term medication for frail and elderly patients. The position focuses on ensuring safe and effective medication use, reducing polypharmacy, and promoting best practices in medication management, all while complying with regulatory standards.

Key responsibilities include advising the frailty team on medication management, supporting care home medication oversight, responding to patient inquiries, and conducting practice-based audits to drive improvements in clinical care.
