Ordsall Health Surgery is a forward thinking and friendly training GP practice with approximately 12,000 patients. We are looking to recruit a motivated and approachable Practice Nurse to join our dynamic and supportive team.

Experience with chronic disease management, women's health, child and adult immunisation and health promotion is desirable.

- Delivering face to face and telephone patient care
- Assess, plan, implement and evaluate treatment programmes that promote health and well being for our practice population
- Identify and manage preventative treatment plans for patients at risk of developing a long term condition
- Work closely with patients and multidisciplinary team members to monitor and treat long term conditions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
-Provide care for our patients with long term conditions such as diabetes, COPD, asthma and cardiovascular diseases
- Support patients and their families to make lifestyle changes that promote the best health outcomes
- Support and manage health needs of women presenting for family planning and cervical screening
- Support the provision of vaccination programmes for adults and children, including for those with travel health requirements.
- Provide care where needed as part of the practice home visit service
Summary of the post
The post holder is responsible for the delivery of general practice nursing to the practice population. This includes the delivery of evidence- based practice for patients with long term conditions and the provision of preventative healthcare. As an autonomous practitioner the post holder will demonstrate critical thinking and skills in clinical decision-making in the management of patients. Working in accordance with the NMC Code of Professional Conduct (2, they will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policies and procedures, and provide nurse leadership and support as required.
Clinical Responsibilities
Review patient needs and treatment holistically in accordance with evidence-based practice and national and practice protocols
Individualise care and involve patients in decisions to support the delivery of person-centred care
Prioritise health problems and intervene appropriately to assist in complex, urgent or emergency situations, including initiation of effective emergency care
Support the care and review of patients in relation to asthma, COPD, diabetes, hypertension, cardiovascular disease, contraception, cervical cytology, adult and childhood immunisations, weight management, mental health and learning disabilities.
Work together to promote patients to live healthily and apply principles of self care
Provide care provision for housebound patients
A strong commitment to working within and promoting best practices in safeguarding both children and adults
Communication
Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment
Maintain effective communication with practice colleagues and the wider primary and secondary care teams
Maintain confidentiality and GDPR requirements
Use a range of clinical information systems to record and communicate care planning, delivery and evaluation (e.g EMIS, Docman, Ardens, Clarity Teamnet, accurx)
